Rate each on a scale from 0 (very seldom) to 2 (almost always):

  5. I feel a sense of purpose in my life.


  6. I am comfortable discussing my personal values and beliefs without shame.

  8. My values and beliefs align with my daily actions or decisions.


  9. I have a mindset that allows for growth and change.


  10. I am comfortable with who I am, and with my spiritual journey.
